Facial topography: reading the face in light and shadow
Reading the face as a surface of lights and shadows, and why that reading determines where volume should go.
Patients describe faces in terms of lines and hollows. What they are actually responding to is topography — the pattern of light and shadow produced by a three-dimensional surface — and learning to read that pattern is more useful than cataloguing individual wrinkles.
Lights and shadows
A convex surface catches light; a concave one falls into shadow. A young face has a characteristic arrangement: highlights over the forehead, the malar eminence, the tip of the nose, the chin and the cupid’s bow; soft transitions between them.
Ageing changes the arrangement rather than adding blemishes. Convexities flatten as fat compartments deflate and bone resorbs. Boundaries between compartments sharpen as tissue descends against tethers. What was a smooth gradient becomes a defined edge, and the eye reads edges as age.
Why this reframes the plan
A patient points at the shadow beneath their eye and asks for it to be filled. Read topographically, that shadow may be:
- a concavity at the orbital rim from bone resorption — a genuine volume deficit;
- an edge created by descent of the cheek below a tether — a support problem;
- a shadow cast by a convexity above it, where orbital fat bulges forward — not a deficit at all, and made worse by adding volume.
All three look identical in the mirror. They require completely different responses, and only the first is treated by filling the thing the patient is pointing at.
Assessment that reveals it
Change the lighting. A shadow that disappears under diffuse light was a topographic artefact of directional light; one that persists is a genuine contour change. Look from oblique angles rather than only frontally. Assess upright as well as supine, since tissue redistributes. Observe in animation.
Photography under consistent, directional lighting is more informative than flat clinical lighting precisely because it exaggerates the topography you are trying to read.
Product follows the reading
Convexity that has flattened wants volume with enough projection to restore the curve — a gel that holds its shape. An edge created by descent wants support at a fixed point. A shadow cast by something bulging wants nothing at all, or a surgical opinion.
The commonest failure in volumising is treating all three as the first.
